Texas posts form for prospective employers to check claim history

The Texas Division of Workers’ Compensation is making it easier for prospective employers to inquire whether a job applicant has previous workers compensation claims by offering a streamlined way to access the information.
Now available online, the Prospective Employment Authorization and Certification form “increases the use of technology by allowing requesters to submit the form electronically,” the division said in a statement.
Requesters will no longer be required to notarize the form but must continue to certify that they are eligible to receive the information. The division also eliminated the $2 fee for prospective employer checks.
