The Occupational Safety and Health Administration on Monday announced $182,293 in fines for a Cleveland bakeware manufacturer that failed to prevent two workers from suffering amputations over unguarded machines.
OSHA investigated the two incidents that led to the amputation injuries and found G&S Metal Products Co. Inc. “did not equip its machinery with adequate guarding or enforce critical safety protocols, including establishing proper lockout/tagout procedures, and failed to provide effective employee training in machine safety.”
The company was cited for one willful violation and five serious violations.
Similarly, the company was cited in 2017 following an amputation, according to OSHA.
